Candidates

Applicants for admission to the university shall submit the following documents:

  • for first-cycle and long-cycle programmes, the original secondary school leaving certificate (Polish Matura certificate or its foreign equivalent) to be taken and certified as a true copy;
  • for second-cycle studies, the higher education diploma and, in the case of candidates where it is necessary to decide whether the field of study completed by the candidate is a related field of study, a supplement or index, to make a copy and have it certified by the university as a true copy;
  • a certificate stating that the school leaving certificate or higher education diploma submitted entitles the holder to apply for, or to continue, higher education in any type of higher education institution in the educational system in which the awarding institution operates. This certificate may be issued by the school or educational institution where the study took place or by the educational authorities of the country on whose territory or in whose educational system the certificate/diploma was issued;
  • a certificate in the form of legalisation or an apostille if the document was issued by an institution operating in the educational system of a State being party to the Hague Convention of 5 October 1961 Abolishing the Requirement of Legalisation for Foreign Public Documents;
  • application for admission to studies - applicant's personal questionnaire, including the applicant's photograph complying with the requirements for photographs for identity cards, printed from the applicant's personal registration account (Note: the status of the photograph "Pending approval" does not affect the printability of the application);
  • an application for an electronic student card, printed out after the foreigner has entered his/her photograph in the personal registration account;
  • confirmation of payment of the recruitment fee in the amount applicable to candidates who are Polish citizens;
  • statement that you are familiar with the amount of fees for educational services in the chosen field of study (a printout from the IRK system)
  • the consent of parents/legal guardians to take up studies, in the case of a minor;
  • a declaration of Polish ancestry, in the case of candidates of Polish origin;
  • proof of knowledge of the language in which the studies will be conducted;
  • documents confirming entitlement to undertake studies free of charge for educational services related to education of foreigners in full-time studies in Polish, i.e.:
  • a document certifying the citizenship of a Member State of the European Union, the Swiss Confederation or a Member State of the European Free Trade Association (EFTA) – a Party to the Agreement on the European Economic Area and their family members residing in the territory of the Republic of Poland, or
  • a document confirming the granting of a permanent residence permit or a long-term resident's EU residence permit, or
  • a document confirming the granting of a temporary residence permit due to the circumstances referred to in Article 159(1) or Article 186(1)(3) or (4) of the Act of 12 December 2013 on foreigners, or
  • a document confirming the granting of refugee status, temporary protection or subsidiary protection on the territory of the Republic of Poland, or
  • a certificate of proficiency in Polish as a foreign language of at least C1 level, issued by the State Commission for the Certification of Proficiency in Polish as a Foreign Language, or
  • A Pole’s Card or a decision on a statement of Polish origin, or
  • a document proving that you are a close family member (spouse, parent, grandparent, child, grandchild) of a person of Polish nationality;
  • a document confirming the granting of a temporary residence permit in connection with the circumstances referred to in Article 151(1) or Article 151b(1) of the Act of 12 December 2013 on foreigners, or a document confirming that a foreigner is staying on the territory of the Republic of Poland in connection with the use of a short-term mobility of a scientist under the conditions specified in Article 156b(1) of the aforementioned Act, or a national visa
  • issued for the purpose of scientific research or development.

 

 

 

Additional documents

  • in the case of a secondary school graduation certificate or a diploma issued in a country with which Poland has not concluded an agreement on the mutual recognition of education documents and in the case of a document from a country with which Poland has concluded the abovementioned agreement, but the document is not covered by it, the foreigner submits a certificate of nostrification or an administrative decision issued by the competent education superintendent confirming the right to apply for admission to studies.
  • confirmation of the degree of pedagogical or teacher training (e.g. diploma supplement, student index) - applies to candidates for second degree programmes in speech therapy and pedagogy;
  • a document showing the grade scale, if it is not indicated on the secondary school leaving certificate or the higher education diploma, certified by:
  • the secondary school from which the applicant graduated, in the case of applicants for admission to first-cycle and long-cycle programmes;
  • the university from which the applicant graduated, in the case of second-cycle applicants.
  • occupational medicine physician’s certificate
  • in the case of applicants for admission to first-degree studies in the following programmes: agroforestry, analytics with molecular diagnostics, biology, forensic biology, chemistry, dietetics, waste management and recycling, computer science, instrumental chemical analysis, process engineering, cosmetology, criminology, nursing, medical rescue, agriculture, zoopsychology with animal therapy, zootechnics - a referral is issued by the admissions committee. The referral must be delivered to the Faculty Dean's Office by the deadline specified in the admissions information;
  • in the case of persons applying for admission to second-cycle studies in the fields of biology, chemistry, dietetics, computer science, nursing, agriculture, zootechnics - a referral is issued by the admissions committee. The referral must be delivered to the Faculty Dean's Office by the deadline specified in the admissions information;
  • confirmation of hepatitis B vaccination for the cosmetology programme. A photocopy of the document must be submitted to the Dean's Office of the Faculty of Medical and Health Sciences by the date specified in the information on admission to the study programme;
  • foreigners applying for admission to study should present for inspection:
  • passport or other document on the basis of which proof of identity can be made;
  • a document confirming the legalisation of residence on the territory of the Republic of Poland.
  • a foreigner with disabilities applying for admission is required to report in person to the Support Centre for Persons with Disabilities to complete a questionnaire and submit a document proving their disability;

NOTE!

All documents issued abroad must be translated into Polish by a sworn translator entered on the list of sworn translators kept by the Minister of Justice or drawn up or certified by the relevant Polish consul.

Candidates are obliged to deliver the required documents by the deadline specified in the recruitment schedule to the Recruitment Centre for Foreigners at 17/19 Żytnia Street, room 107, 1st floor (Student Dormitory nr 4 in Siedlce).

Documents are accepted from Monday to Friday between 8am and 3pm.

IMPORTANT! If you send your documents by post or courier to the above address, please remember to enclose the complete set of required documents, including the original secondary school leaving certificate or higher education diploma.

Please be advised that in the event that the originals are not provided, an administrative decision will be issued to refuse admission.

In the case of candidates applying to study in English, the applicant sends scans (in English or Polish) for initial verification:

  • secondary school leaving certificate (Polish Matura certificate or its foreign equivalent) or higher education diploma (with legalisation or apostille) with a note or as a separate document stating that the certificate/diploma entitles the holder to apply for admission to or continuation of studies at any type of higher education institution in the country in which the certificate/diploma was issued. This certificate may be issued by the university/educational institution where the study took place or by the educational authorities of the country on whose territory or in whose educational system the diploma was issued;
  • supplement or index;
  • English language certificate.

*NOTE!

  • A student can only have one student ID card. Persons who hold a student ID card issued by UPH (e.g. students or this year's graduates of UPH) do not submit applications for ELS (Electronic Student ID card).
  • The fee for the Electronic Student Identity Card must be paid at a later date, specified in the information on enrolment, only to the individual bank account number generated in the USOSweb system at https://usosweb.uph.edu.pl/.
  • PLEASE DO NOT PAY YOUR ELS FEES INTO ACCOUNTS TO WHICH YOU HAVE PAID YOUR ENROLMENT FEE - THESE PAYMENTS CANNOT BE CREDITED.
  • In order to use the USOSweb system, you must use your e-mail address as your ID and the password you used when registering in the IRK system. Persons who are already previous UPH students log in the USOSweb system using their existing data.
